What is a (good) Failure Modes and Effects Analysis (FMEA)?
What exactly IS a FMEA (Failure Modes and Effects Analysis) and what does a 'good' one look like? Good FMEAs are those that achieve a specific outcome. And that outcome is always to improve the product, system or process they are focusing on. Analyzing problems is very different from solving problems.
